Our Commitment to Your Privacy

At Blossom Pediatric Dentistry & Orthodontics, protecting your child’s health information is important to us. We are required by law to maintain the privacy of protected health information (PHI), provide you with notice of our legal duties and privacy practices, and follow the privacy practices described in our Notice while it is in effect.

Our Notice of Privacy Practices explains:

  1. how we may use and disclose health information,
  2. your rights regarding that information,
  3. and our responsibilities for safeguarding it.

We encourage all parents and caregivers to review this notice carefully.

Notice of Privacy Practices

This notice applies to all patients of our practice and includes information about additional federal protections for certain health records, including substance use disorder records, as required by law.

Paper copies are also available at our front desk upon request.

Special Protections for Certain Health Information

Some health information, such as records related to substance use disorder, mental health, genetic information, or HIV status, may be subject to additional confidentiality protections under federal or state law.

Our Notice of Privacy Practices includes information about:

  2. limits on its use and disclosure,
  3. and patient rights related to privacy and nondiscrimination.
